The RECONNECT service was commissioned by the NHS to supports individuals who are released from prison with physical and/or mental health or substance misuse needs, who are unlikely to engage in services alone.

- Do you need help registering with a GP, need reminding of appointments and engaging with local services?
- Would you like a Reconnect worker to support your link in with local services?
The Forward Trust Reconnect Service offers transitional support to Service Users who may struggle accessing and engaging with community healthcare services after release.
